EFFECTIVE 12/6/23 through 2/1/24 we will have a temporary hold on new patients. Exceptions will be newborns and siblings of existing patients. This is so that we can ensure we have plenty of "sick appointments" available to our existing patients. We appreciate your understanding. We will be accepting paperwork, however appointments will be scheduled for after 2/1/24.
To get started please fill out the paperwork below and send us a copy of both sides of your insurance card(s) and photo ID so there is no delay in establishing care and making an appointment. In order to ensure we are contracted with your insurance and there are no insurance issues, we will not be able to make an appointment until we have all of the above documents.

Please remember it is your responsibility to make sure we are in network with your plan.
(This is different then just being contracted and may result in additional out of pocket charges)
